As a new mom I have been poring through the literature on childhood vaccines and comparing the CDC with the EU (@vp- at least the EU has a website where you can quickly compare diff vaccination schedules across countries by the name of the vaccine). It is a daunting task to figure out do we really need Hib or PCV or Varicella or Hep B or Rota or the new Beyfortus (vaccines that weren't around when I was a baby).. and so we really need the recommended number of doses for Tdap and MMR. we finally decided on following the Danish schedule with small tweaks with our pediatricians blessing. It irritates me that a second dose of MMR is given because a small fraction of kids don't develop immunity with the first dose... where is the evidence that giving a second dose improves "real" outcomes for ALL kids. How come Tdap is only 3,5,12 in Denmark but 2,4,6,18 in the US? Some level of constant randomization on these topics to figure out non inferiority would be so useful. And sure, we can't say anything about autism because the Scandinavian obs study showed a null effect with MMR but what about the whole vaccination schedule including literally all of the 21 inoculations given before 1 year? And what about the sky high rates of autoimmune diseases in the US vs other countries... if no one is looking for this safety signal I just wish more pediatricians would directly recommend a European schedule. I started following Dr. Elisa Song, I don't remember exactly but I think she said the mumps portion of the MMR shot is useless, and it might be hard to find but you can ask for separate shots of the other two components. I enjoy the chemistry you docs have together, and I think we are intellectually challenged best by colleagues like you who have disagreements and can debate each other. I am personally most aligned with Dr. Cifu (I had the great honor of meeting him when he visited our medical center a few weeks ago), but I have come to really love the passion and courage of Dr. Prasad, who reminds me of my family members, and Dr. Mandrola's brilliant deep dives into major studies. I suppose you have all helped me gain more critical understanding of some big weak spots in high-profile studies.

Screening for most cancers is probably not cost effective, and has not be shown in quality studies to reduce overall mortality (I'm repeating what I've learned on Sensible Medicine here). The major new therapeutic agents in oncology are approved by the FDA based on studies showing benefit in surrogate endpoints, not the endpoints that matter. Quite a bit of oncology seems to be based on burning huge amounts of cash to kill cancer patients a little faster than their malignancies would, left unattended. There are dramatic exceptions to this, of course; but too much money is spent approving and utilizing agents that simply don't work on hard outcomes. Will RFK Jr's crusade against the FDA improve this situation? I would like to be optimistic about this, but it's difficult to see how he could pull this off.
Cynically speaking, however, I might posit that primary care practices like my own might become more interesting. Other than teaching, most of what I do is data entry and the ordering of medications and tests to meet institutional metrics, so that the bean-counters will leave me in peace. It will be much more stimulating to my students and I when we have the opportunity to diagnose and treat mumps, diptheria, polio, tetanus, and hepatitis A again. A case of lockjaw in clinic will be a fantastic teachable moment. Perhaps I will even be able to leverage new outbreaks of old diseases into mentoring a student that chooses to go into outpatient internal medicine! That, certainly, is an event even more rare than finding a "brain worm." And I'd love to see more worms escape RFK Jr's CNS and get out into the general population...then we can finally start prescribing ivemectin in good faith!
There are good and bad parts of the MAHA movement.
Unfortunately, the best parts of the MAHA movement are unlikely to gain traction with the new administration. I doubt MAGA will support limits on the pharmaceutical industry and forbidding FDA officials from taking jobs with the industry. MAGA will take credit for drug cost reductions the current administration negotiated that will take place next year but I doubt there will be further cuts ("industry needs incentives to develop new drugs"). I doubt they will put limits on ultra processed food as that requires fighting the food industry. MAGA is too pro industry to take them on. Also there is a market for junk food because it is addictive. I can hear the wails "they want to take away my Cheetos". Remember how Michelle Obama was ridiculed for wanting to improve school lunches? I hope I am wrong and MAGA will accept change if it is suggested by someone they perceive as being one of their own. But I am doubtful. More likely the new administration will only support the worst parts of MAHA such as getting rid of vaccine mandates in school. Although this is controlled locally, Trump has threatened to withhold federal funding to schools that mandate vaccines. And will taking fluoride out of water or other undesirable actions become policy? And banning windmills since they cause cancer? (and are killing all the whales).
I’m with Vinay and will remain optimistic until proven otherwise. In addition, he will be working with Dr. Casey Means and others. Totally agree that revolving door politics needs to be discontinued. In addition he’s asking for more EBM studies and strengthening drug surveillance.
As far as raw milk, that was one of the main issues that mobilized the large Amish population in PA to vote in this election.

I’m with Vinay and will remain optimistic until proven otherwise. In addition, he will be working with Dr. Casey Means and others. Totally agree that revolving door politics needs to be discontinued. In addition he’s asking for more EBM studies and strengthening drug surveillance.
As far as raw milk, that was one of the main issues that mobilized the large Amish population in PA to vote in this election.
I think the antivax claims about him need to be dialed back and I encourage everyone to actually dig in to his history by him not the MSM version. He has said he wants more studies.
In addition we have so much industry capture in our food system and I am excited to see what his team can do.
I’m not going to bash a plan that hasn’t even had a chance to be put into action. Give these people a chance and be a part of the solution by getting involved.
