Many of us in the health professions have been raising these concerns, but I don't think we fully grasp just how deeply corporate monetary interests have compromised sound health policy. Congratulations on your recent nomination! If confirmed, I hope you'll seriously consider taking steps to remove high fructose corn syrup (HFCS) from our food supply. I recently wrote an article explaining why this is critical, but I'm sure you're already aware of its impact. I believe this single measure could lead to an immediate reduction in the obesity crisis.

I have thick skin having faced so much push back and resentment trying to reverse T2D and obesity...but my skin getting thinner :(
I wouldn't agree that emergency care runs like a whistle, but I take your meaning that the system is designed for acute care and for everything else we are solving the wrong problems in the wrong way. In addition to nutritional and environmental issues, I think emotional/stress based health issues are poorly understood and poorly managed. I also think that many people go into medicine to prove to someone that they are "good". Pair that with shame and fear-based education and I think people get conditioned to blend in and incredibly uncomfortable with the threats and uncertainty that comes with being labeled unprofessional that inevitably happen when you challenge the existing system, even though we all know it's shit.
Marty is a smart guy and his first book was great. But, at least in this excerpt, he makes claims, that as trained epidemiologist, he knows are flimsy and unproven. For example, the study on Cesarean section and colon cancer is observational and can't prove causation. Obviously, an RCT on this question is impossible. Much of what he says is true or at least plausible, particularly on our food and diet, but it seems to me that many of his claims are far from proven yet. Still, I think he does a good asking the right questions.
I looked briefly at the linked study on c-sections and colon cancer. They found no association for the overall group and no association for males. When they looked at females, they found a small association. To site that article in a suggestion that c-sections are related to early onset colon cancer seems a stretch at best. It seems to me that the article was stretching to find any "positive" outcome because "negative" outcomes don't get attention.
Right action can only be taken in the present moment, which is now. Individual collective actions which are in alignment with humanity and our peaceful survival must guide new incentives and systems. We can't survive the alternative - which is all we're offered today by an economically captured system. Awakening to truth and transparency in our shared present moment is necessary, but not sufficient. Awakening is simple, but of multi-step and long process. Every step counts and Dr. Makary's voice of empathy and humanity provides more information and evidence to drive additional moments of awakening across society. We common people are all we have. Nobody else with money or power is going to come save us, as we can see by the abysmal condition of our failed empire.
But up front I will say I'm tired of whining about US healthcare when it is not accompanied by solutions, or 'solutions' that point the finger at "Big Pharma", or greed, or agribusiness. We can't go back to all growing our own food folks and we can't 'ban capitalism".
And US healthcare is the envy of the world in many ways, as evidenced by the hundreds of thousands that come here every year from other countries to be treated.

The same could be said for salt or fluid restricted diets in HF patients. Medicine is replete with axioms that have no evidentiary base.
