1 - lately I have not had time to read these posts as they arrive in my mailbox, but I save them all to read later.

2 - I appreciate being able to speak to some posts despite my credentials being RN and not MD. Here is where I can reflect on my education, who I used to be as a new nurse, and how each of you enrich my medical knowledge as I grow into a new RN yearly, which in turn helps me better care for my patients.

3 - For what it's worth, I love your reflections on being pre-med, med student, and residency.

4 - I lean toward Vinay's writings, thoughts, opinions, etc. as I'm pretty sure that would be me if I had gone through education and licensing as a doctor. (sometimes this revelation scares me)

Thank you! I enjoy seeing the different perspectives of all 3 of you as well as the guest contributors. It is refreshing to have a place where I can see fellow physicians speak plainly, argue respectfully & teach us all more about research & evidenced based medicine. Cheers to more years!

I always feel bad when I am too busy to read all of your essays. In addition, I do not always understand the studies you reference or how randomized controlled trials work. But I do appreciate your essays. One thing that stands out is the reference to people hate doctors. I think it it more that people hate the system. I ended up in the ER this week due to a horrible nosebleed. They stuck a cotton wad up my nose. It stopped the bleeding, but they did not tell me I needed to get my blood pressure down.

They did send me to an excellent ENT doctor. His advice: Don't go to the ER for a nosebleed. Keep Afrin on hand, put it on a cotton ball and put that in your nose. And keep my blood pressure down This doctor is the kind of doctor you respect. Common sense that you cannot find on the internet.

Did I read correctly, that the three of you are going to meet in person for the first time?

I appreciate all your medicine related input and have learned a lot! I don’t live in the US but even home I don’t usually engage in political discussions. Social media has unfortunately unleashed bullying at a level we wouldn’t see if you could not hide in an avatar. For me, who were born in El Salvador, studied medicine in Guatemala and have a private practice in Sweden, it is amazing how this profession (or Call) transcends countries, creeds, religions and everything that may divide us and unites us in the goal of being a better doctor despite many years of practice. Thank you very much for your generosity.

You all write with a candor and honesty that are refreshing and rare.

But as JM notes, editing is hard; the typos and grammatical missteps are frequent.

At 93,000 subscribers, perhaps you'll hire an editor?

Congrats gents. This stack is mandatory reading for me, to improve my knowledge, to hone my appraisal skills, and to broaden my medical perspective. It’s a welcome respite from the cheerleading and collective grandstanding that passes for neutrality that I see from most other medical sources.

Nice synopsis. I started reading and then subscribed, initially because I liked your interpretation of cardiology studies, but too often now, I feel your lean on interpreting data has a political bent that I just don't want ...I got it, you're angry about the Covid response, RFK ain't so bad...so I hope you all are correct that something good will come out of this chaos. I'd prefer more discussions of medical studies without the political messages. And thank you for letting me guest post when I didn't agree with one of your contributors. Enjoy your anniversary, back when I had a blog with 3 psychiatrists (complete with trolls), we used to have in-person celebrations. Oh, long after our 12 year blog ended, we still get together to eat, often inviting the editor we've shared with you. Best wishes and Thank you.

Congrats and thank you!! Perhaps hate is a strong word to describe the feelings of some folks towards physicians. Seeing it written in that way creates my own self reflection. It's not hate as much as it is wariness or fear of coming out of the encounter with an iatrogenic harm that I must deal with temporarily or permanently. I am hoping my physician earns my trust and doesn't demand it. Earns it through communication and the making shared decisions based on my particular history (and not the "guideline") that help me stay healthy.

Congrats and thanks to all three of you on this endeavor! you encourage me to think even more critically, no matter my bias. and i admit to being a curmudgeony, biased, retired L&D RN. when i think of the three of you together, i picture the see no evil, hear no evil and speak no evil primates. you pick who's who....with the exception of Vinay speaking no evil. 😉 Please keep bringing us the goods...i can hardly wait to devour the substack when it drops.
