Regarding the code issue, the main algorithm is shared and written in the paper, but I can tell you, many of the results would not be reproducible without the source code in many studies. And of course you also have coding errors that no one is checking for. Most likely no one will try to reproduce the results in any case, for the vast majority of publications.
Additionally many authors will propagate those errors in many papers due to their specialization within their field to a type of model / simulation / code...
An of course there are methods to correct those errors and some models are widely used and they've been checked many times, but we are not infallible and there may also be intent (i.e. Mr. Ferguson).
